The Experience

In your one-hour initial evaluation, our highly qualified therapists will assess your posture, movement, gait, joint motion, and strength to identify faulty patterns that are causing or perpetuating your pain. Once the therapist has identified the source and cause of your pain, you will be given specific strategies to eliminate it. Some of these may involve changing your posture or movement behavior, others may involve specific relaxation techniques (massage, breathing) or strengthening exercises. In one-on-one 45 minute sessions, you will receive hands-on manual therapy. Our therapists will help to re-align and mobilize your joints and release restrictions that are contributing to your pain. When strengthening exercises are necessary, you will be trained by your therapist to perform them efficiently and then asked to repeat some at home on your own. Your sessions will take place in KIMA's private treatment rooms or spacious exercise studio.

Orthopedic Physical Therapy

Orthopedic Physical Therapy is a branch of medical science that deals with the prevention and correction of disorders involving loco-motor structures of the body, focusing on the skeleton, joints, muscles, fascia, and other supporting structures such as ligaments and cartilage.

KIMA specializes in orthopedic physical therapy and uses the very latest manual therapy techniques, in addition to evidence-based therapeutic exercise prescription. This hands-on approach combines osteopathic joint mobilization with soft tissue techniques (myofascial release, functional mobilization, edema management, neural release) and targeted prescriptive exercise.

We provide individual therapeutic programs tailored to each patient’s specific limitations and goals. We use research-based exercise approaches which train the local, deep muscle system of the body to provide stability and support. We train the global non-weight bearing and weight-bearing muscles of the body to optimize muscle control and power. We are proud to be one of the first practices in Manhattan to utilize real-time ultrasound biofeedback imaging for teaching deep muscle activation, a technique that was developed by world renowned researchers in Australia.

KIMA places a high priority on individual attention and one-on-one treatment sessions. A comprehensive evaluation will be performed and a clinical reasoning model will be used to determine the specific source and cause of your symptoms. Traditional modalities such as electrical stimulation, ultrasound, ice, and moist heat therapies are offered in conjunction to our hands-on treatment.

Sports Rehabilitation

KIMA provides physical therapy and training for amateur and professional athletes including tennis players, golfers, dancers, Olympic athletes, and marathon runners. We assess aerobic fitness, muscle endurance, gait, and power. Through specific biomechanical analysis and assessment of flexibility, form, technique, and strength, we determine the source and cause of your pain, or the physical limitation that is preventing optimal performance. The goal is to eliminate your pain and restore your movement, as well as to address faults in muscle balance and general strength. As your rehabilitation progresses, we address sport specific strategies, maximize strength, and power to develop explosive force and speed. An individualized program will be prescribed to include periodization, a realistic time frame for improvement, and measurable goals and objectives.

Tennis

KIMA offers an integrated service for tennis players of all levels. Our therapists work with professional tennis players throughout the year at the U.S. Open and Fed Cup as the primary tournaments. We have developed a thorough program to get athletes back on court and keep them playing at their highest level after injury. A thorough biomechanical evaluation of the serve, strokes, form, and playing technique is essential to identifying the cause or contributing factors to pain, injury, or poor play. An assessment of flexibility, strength, power, and control is crucial to devising a specific program to restore optimal play. In collaboration with your coach or tennis instructor, a therapist will link your physical limitations with your technical faults.

During the last 20 years the game of tennis has intensified and become more physically demanding. It has progressed enormously with new playing techniques and changes in racket design. The players are now stronger and faster than ever before while advancements in equipment technology have enabled players to produce velocities over 150mph. Athletes are playing more from the back court and with open stances that permit the generation of greater forces. Adding more power to the game has also increased injuries.

Considering the demand of today’s tennis game, we focus on: Flexibility, Control and Power.

Flexibility: It is necessary to maintain flexibility in your legs and spine to support an efficient tennis technique. Through manual therapy we can decrease your joint restrictions, improve range of motion, and improve flexibility in your muscles and tendons. Flexibility is key for rapid bursts and deep lunges when, for example, you have to return a low volley. Accelerating after an abrupt stop is also a necessity for competitive edge. As in a spring, if you have good flexibility and recoil, you can propel at much higher speeds and load your joints less. We identify your limiting factors to develop this crucial aspect of your game.

Control/Stability: Most shoulder, hip, knee, and spine injuries in tennis are due to the lack of balance and control within the muscles surrounding those joints. Focusing on balancing your muscle strength throughout the necessary ranges of motion restores control to the most important joints for tennis. Training the deep, local muscle system of the body gives you control and swiftness, allowing you to achieve a higher sense of bodily awareness for improved play. To assist you in activating these deep muscles, we use Real Time Ultrasound Imaging as a biofeedback tool. Roger Federer is an example of a player mastering the art of control.

Power: By combining elements of flexibility and control we are able to achieve higher levels of power in your game. Once we restore efficient patterns of movement, technique, strength, flexibility, and control, you can achieve explosive, powerful movements on the court. Continual assistance, through manual therapy, biomechanical analysis, and playing technique re-education will assure a constant growth of power in your game to optimize your play.

Golf

KIMA offers an integrated service for golfers of all playing levels. Our aim is to prevent and eliminate pain as well as to enhance playing performance, longevity, and enjoyment. Specific assessment of the golfer's posture, flexibility, stability, strength, and power will form the foundation of his/her program. With the golf coach or instructor, we will link physical limitations to your swing faults. KIMA therapists stay on the cutting-edge of golf rehabilitation and are closely associated with golf schools such as Jim McLean and researchers and golf specialists such as The Golf Athlete, Golf Biodynamics, and FitforeGolf. We hold courses annually at our 23rd street location to train other therapists to work with the golf athlete.

Posture: Many of the postures that are assumed in the work environment may be detrimental to you as a golfer. Working in front of computers predisposes the golfer to poor posture, both at address and throughout the golf swing. It is imperative that posture be considered and that the golfer practice ideal posture and gait daily to see pain reduction and technical improvement.

Flexibility: Flexibility in the thoracic spine, hip, and shoulder is essential to the golfer. Swing mechanics and technique, in part, determine flexibility. If it is not sufficient to allow full torso, hip or shoulder range of motion, pain or inefficient power will result. All golfers should have a well structured set of stretches for warm up, injury prevention, and optimal driving distance throughout their playing years.

Stability: The ability to control one’s body when balanced on one leg is key to ideal weight transference in the golf swing. Equally important is the ability to recruit the large muscles of the leg and trunk throughout the swing. Weakness in these muscles can cause uncontrolled movement and increase stress on joints and nerves. An independent routine of strengthening exercises is a priority when the golfer shows loss of control and tests weak in these muscles.

Golfers and Back Pain: Back pain among golfers has almost reached epidemic proportions. Professional golfers have the highest incidence of low back pain compared to all other professional athletes, and back pain is the #1 injury among recreational golfers. Several factors have contributed to the increase, but irrespective of the cause, pain can be prevented, controlled, and eliminated. All it takes is some guidance and understanding, which is why we use the comprehensive Five Stage Back Care Program that was developed by world renowned golf researcher David Lindsey, BHMS, BPhty, Msc, CAT.

Spinal Specialties

Low Back Pain

It is well known that the lifetime incidence of back pain is extraordinarily high, affecting over 80% of the population. Low back injuries heal and the majority of people (50-80%) recover within 6 to 8 weeks. Others go on to develop recurrent and chronic low back pain. Long-term healing is difficult to achieve, and without the correct treatment, 4 out of 5 people will suffer a recurrence within one year.

The right treatment can make all the difference between healing completely and developing recurrent episodic low back pain that becomes chronic, leading to long term dependency on pain medications and even surgery. At KIMA, we believe that the right treatment depends on a thorough, subjective discussion with you and an objective physical assessment to identify not only the source of your back pain, but what is driving the disorder. Our therapists aim to treat the cause of your back pain, instead of only using modalities like moist heat and electrical stimulation to temporarily mask the pain.

There is a difference between the source of your pain and the cause of your pain. A specific source of your back pain may be a herniated disc or an irritated nerve such as your sciatic nerve. Sources can be treated with anti-inflammatory and pain medications, or modalities such as ice, heat, electrical stimulation, massage, and acupuncture. If your symptoms progress, your doctor may also try trigger point injections, epidurals, radio frequency denervation (heating of the small nerves that innervate the joint), or surgeries (for example micro-discectomy, laminectomy, disc replacement or spinal fusion). Although many people get relief from these procedures, others continue to experience pain. Sometimes it is not enough to only treat the source of the pain, but it is also necessary to identify the cause.

Some specific causes of low back pain are

* Habitual standing (i.e. swayback) and sitting postures (i.e. slump)

* Inability to move the lumbar spine independently from the hips and thoracic spine

* Excessive uncontrolled movement or immobility at specific joints

* Poor gluteal (buttock) and multifidus (back muscle) strength and control

KIMA therapists will help you to understand your back pain differently. We will help you to figure out what is causing your symptoms and how to eliminate them. Our highly qualified therapists will assess your posture, movement patterns, joint motion and strength, and identify faulty patterns that are causing or perpetuating your back pain. Correcting these movement patterns and tissue adaptations will not only alleviate pain, but prevent it.

Once a therapist has classified your low back pain, you will be given specific strategies to eliminate the cause. Some of these strategies may involve changing your posture or movement behavior, others may involve specific relaxation techniques (massage, breathing) or strengthening exercises. KIMA offers real time ultrasound biofeedback for patients with low back pain who need to activate important muscles in the spine which have atrophied or are not under the patient’s control.

Neck Pain and Headaches

Neck pain is a common complaint with a lifetime prevalence of nearly 50%. There is significant evidence to support the use of manual physical therapy techniques and therapeutic exercise to prevent and eliminate neck pain (see articles listed below). At KIMA we use the latest orthopedic assessment and manual therapy techniques to treat neck pain. Some of these techniques include joint mobilization of the cervical vertebra that may become stiff and make it difficult for you to turn your head. Other techniques involve the strengthening of the deep neck flexors that have been proven to weaken in individuals with neck pain, headaches and/or whiplash (Jull 2000) (1). These muscles have also been proven in randomized control trials to reduce pain and restore neck function (Jull et al 2002) (2). At KIMA, we will help to identify the underlying source of your neck pain and the factors that may be perpetuating it.

Headaches in some form are experienced by at least 90% of the population and 70% of them are associated with neck pain. At KIMA, we specialize in treating cervicogenic headaches, caused by dysfunction in the cervical spine/ neck. A detailed subjective and objective examination will help our therapists determine whether your headaches are of cervical/ neck origin. There is significant evidence as to the efficacy of physical therapy management of cervicogenic headaches (Jull et al 2002) (2). These techniques will be administered in KIMA’s private treatment rooms and prescribed as a home exercise program with the goal of eliminating your neck pain.

Hip Specialty

The hip is a common sources of musculoskeletal pain and dysfunction. It can be isolated or, in many cases, may coexist with low back pain. Pain from the hip can refer into the groin, knee, or pelvis. Pain from the low back can refer into similar areas and often can be difficult to differentiate from hip pain. At KIMA, we specialize in treating this complicated area and will work with you and your physician to understand your pain and eliminate it.

Hip pain can come from many different sources but most commonly is referred from the joint (i.e. osteoarthritis), the labrum, condral injuries, the capsule, muscles, and nerves. Once the source of your pain has been identified, the therapist will look for underlying causes of your pain, like posture, muscle imbalances, joint stiffness, and loss of range of motion. Specific joint mobilization and soft tissue techniques will be used to decrease your stiffness and optimize the position of your joints.

Recent research has identified very specific atrophy of the deep muscles of the hip in patients with hip dysfunction. These deep muscles provide a very important stability function and when they are inactive or weak this can cause significant muscle imbalances around the hip and pain in the superficial muscles. These imbalances can cause clicking and snapping inside the hip and further aggravate the source of your hip pain. At KIMA we will address these imbalances and teach you how to activate the deep hip muscles as well as balance the painful superficial muscles. To assist you in activating these deep muscles we use Real Time Ultrasound Imaging as a biofeedback tool.
